Mandatory electronic declaration form
Travelers are required to complete The Guam Electronic Declaration Form (Guam EDF) up to 72 hours before arrival in Guam.

Mandatory health declaration form
Travelers arriving at the Antonio B. Won Pat International Airport must fill out the Health Declaration form for themselves and all family members who are traveling together on the same flight. Information collected on this form will be used to enroll travelers into the Sara Alert system for symptom monitoring.
